The Spring Center employs a hybrid billing model.
We bill qualifying insurance and also have out of pocket costs for non-billable services
Our new patient intake process is fully comprehensive and covers family history, social & environmental history, health concerns, and preventive care. Your first step in this health journey is to become a confirmed patient of The Spring Center.
New Patient Package WITH Qualifying Insurance* $2100

1) Initial Lifestyle Consult
MUST BE IN OFFICE
45 Minute In-Person with our Physician Assistant to explore your medical history through a Functional & Integrative lens & focus on 4 contributing lifestyle factors: Nutrition, Sleep, Stress, & Exercise.

2) Physician Appointment
MUST BE IN OFFICE
60 Minute In-person Physician Consultation to address health concerns and formulate initial treatment plan.

3) Physician Assistant Follow Up
TELEHEALTH ALLOWED
30 minute appointment to review initial labs and augment treatment plan.

4) Functional Nutritionist Appointment
TELEHEALTH ONLY
40 minute virtual appointment with our Functional Nutritionist to curate your personalized nutrition plan.
During your initial consultation, you and your practitioner will create your treatment plan which will include innovative lab testing, dietary adjustments, body movement/exercise, supplementation, and other lifestyle changes.

Please note, you must complete your 4 appointments in the New Patient Package prior to moving on to follow-up appointments. Completing the New Patient Care Package takes approximately 8 weeks.

Insurance
We accept some insurance. We do have additional fees that allow us to offer you the wide range of specialized services not available at other clinics that may not be covered by insurance.
As a courtesy, we bill most PPO insurances and Medicare for a portion of the office visits. We do not perform benefit verification, therefore, we ask that prospective patients become familiar with their insurance plan(s) and check with the plan to determine if we are in- or out-of-network.

It is the patient’s responsibility to ensure we have accurate current insurance information on file. We strongly encourage patients to confirm first with their plan that we are in-network.
We use a variety of testing that may or may not be covered by your insurance plan. Proceeding with any recommended testing or treatment is up to the sole discretion of the patient.

- Aetna MemorialCare plans (or any insurance company plan through MemorialCare)
- MediCal
- Optum (includes plans through companies like Anthem, such as Blue Open Access POS)
- CalOptima
- Tricare (as primary insurance, exception may apply, contact Tricare)
- Cigna Local Plus PPO plan
- Anthem EPO and Anthem Select network (including PPOs)
- Any Healthshare plans (Liberty Healthshare, Christian Care Network, etc.)
- Oscar
- O’NA Healthcare
*This information reflects our current knowledge of the insurance landscape but is constantly changing. It is the responsibility of the patient to be familiar with their insurance plan benefits, and to confirm whether we are in-network with their plan.
Additional Costs
In addition to non-billable services, patients may also pay for IV nutrients and materials, supplements, lab kits, lab fees, and other miscellaneous items. Some of the labs we work with can bill insurance for tests, but we ask that patients first verify coverage for their ordered tests prior to having the labs done. We strongly encourage our patients to become familiar with their insurance coverage, as with the thousands of plans, it is impossible for us to check each one for every patient.
Some lab fees are strictly CASH only (out of pocket). For labs that don’t accept insurance, fees will be provided, but are subject to change. For most up-to-date costs, we advise patients to contact the lab directly. Depending upon your health goals, and insurance coverage, additional fees vary greatly.
Please note that testing is not performed in our office.
Cancellation Policy
We require TWO WEEK advance notice for initial appointments and 48 business hour notice for all other appointments. We reserve that time specifically for you and do not overbook.
Any cancellation made less than (the above mentioned) in advance will result in a cancellation fee starting at $75. Fee may be as great as $500 depending on the type of appointment or service.
